A patient with long-standing renal disease (Stage 3B-CKD) and never previously on dialysis presents with symptoms concerning for a bulbar stroke. He receives an MRI with gadolinium which showed no evidence of stroke. Three weeks later he presents to clinic with this new rash on his arm. What is the diagnosis?
